IAP offers you a variety of ways to apply for low-cost group insurance coverage...

Apply Online

To complete an online application for Term Life, Dependent Children Term Life, Accidental Death & Dismemberment and/or Critical Illness Insurance, follow these three easy steps:

1. Open the Online Application page.
2. Complete the application in full.
3. Choose to either submit your application electronically or print out a copy and mail it to IAP.

To use IAP's online application, you must have JavaScript and Cookies enabled on your browser. You will also need Adobe Acrobat reader, available free from Adobe, to view and print the application form.

Please note if you do not have existing coverage under this policy, you will need to provide credit card information if you wish to submit your application electronically.

Print an Application

To print out an application, select one or both of the following links:

1. a) For Term Life, Dependent Children Term Life, Accidental Death & Dismemberment, Critical Illness and/or Office Overhead Insurance, open and print the Application for Voluntary Group Insurance form (PDF - 63 KB).
   
   b) For Health & Dental Insurance, open and print the Application for Health & Dental Insurance form (PDF - 75 KB).
2. Complete the application in full.
3. Mail your completed application along with a cheque for your first month's premium, if applicable, to IAP.
